THE PRIMARY SERVER CAN CRASH WITH INVALID MUTEX TYPE WHEN DISCONNECTED WITH RSS SECONDARY SERVER

| Problem description: |
A timing issue between the smxrcv and smxsnd threads can cause
smxsnd to remove a pipe that has already been destroyed by the
smxrcv thread. The server will fail with Invalid Mutex Type and
has the following stack trace:
